The pen format

Handling and storage

Tirzepatide or retatrutide?

Tirzepatide agonises two receptors, GLP-1 and GIP. Retatrutide adds a third, glucagon. That single addition is the whole decision.

GLP-1 and GIP act largely on appetite, insulin response and gastric emptying, reducing what you take in. Glucagon receptor agonism brings energy expenditure into it, so the body spends more as well. A more aggressive mechanism is more aggressive in both directions.

Tirzepatide is the better-characterised of the two by a wide margin, with a larger and longer-running body of evidence. Retatrutide is newer, with trial results that have attracted attention for their magnitude and a correspondingly shorter track record.

If this is a first GLP-1, that evidence gap is a reasonable thing to weigh. Both are supplied in two strengths because these compounds step up gradually rather than starting at the target amount, tolerance to the gastrointestinal effects builds over time, and skipping ahead reliably produces a bad week.
