About SS-31

SS-31, also known as elamipretide and previously as Bendavia or MTP-131, is a four amino acid peptide with an alternating aromatic and cationic structure. That structure is what allows it to cross the mitochondrial membranes and concentrate inside the organelle, which most compounds cannot do.
Once there it binds cardiolipin, a phospholipid found almost exclusively in the inner mitochondrial membrane and essential to the organisation of the electron transport chain. Cardiolipin becomes disorganised with age and with mitochondrial disease, and the research proposition is that stabilising it restores the geometry the respiratory complexes need, improving ATP output and reducing the electron leakage that generates reactive oxygen species.
That is a genuinely different mechanism from an antioxidant. An antioxidant mops up reactive species after they form; SS-31 is described as reducing their formation by fixing the structure that was producing them. Stealth BioTherapeutics has taken elamipretide through multiple human trials in primary mitochondrial myopathy and Barth syndrome, with mixed results across endpoints.

Where the evidence sits: elamipretide is among the most seriously investigated compounds in this category, with phase III human trials completed. Those trials produced mixed results, missing some primary endpoints while showing benefit on others, and it has not obtained approval. The rare-disease populations studied are also not the healthy adults who typically buy it.

At 50mg this pen carries five times the peptide of the 10mg SS-31 pens sold elsewhere, and 25 full 2mg draws. The dial resolves to 167mcg.

Choosing between the SS-31 pen, the vial and the MITO penThe Quanta SS-31 vial holds the same 50mg for less money, and if you are comfortable reconstituting it is the cheaper route to the same compound.
